J2create allows you to create interactive content that can survey user opinions, generating a useful information sheet. 



Using forms


Begin by opening a j2create  file. You might like to populate this with information that will be relevant to the questions. For information on how to use the other functions in j2create, click here


Once your document is populated, click on the "forms" button. This will give you a number of form field options for your form. Your choice will be determined by the information you wish to collect:


  • checkbox - used for multiple answers from a multiple choice question
  • radio button - used for a singular selection from a multiple choice question
  • text box - used for a single line of text from an open ended question
  • text area - used for multiple lines of text from an open ended question
  • drop list - an editable list of answers


Each form field category will provide different functionality, and allow you to create a differentiated survey such as this:


To begin...

Click the forms button and select a form field from the list. As soon as you select a form field the relevant box/button will appear, and float until placed on the page. To place the box/button on the page, left click on the page. 


A "send" button will appear, and float until placed on the page. The "send" button is used to submit the data from the form, and so will need to be placed near the end of the quiz/survey/questionnaire/document. Left click on the page to place the "send" button:




You can now add more form fields onto your page:


Check boxes and Radio buttons


Check boxes allow you to add multiple selection answers to questions, whilst radio buttons allow the selection of one answer from a list. Click the checkbox/radio button and drag a checkbox/radio button onto the page. To change the text, click on the word "checkbox" delete and rewrite the checkbox label. Hit the enter key to rename.

 



Textbox

Text boxes allow users to add a line of text as a response. Click on the text box button and drag a text box answer onto the page. Once placed, the text box submenu will appear on the right hand side, allowing the text box to be tailored to the type of data required:




Text area


Text areas allow users to add multiple lines of text as a response. Click on the text box button and drag a text area answer onto the page. Once placed, the text area submenu will appear on the right hand side, allowing the text area to be edited.



Drop box

Drop lists are an editable list of answers. Drag a drop list answer onto the page. Once it is placed on the page, the drop list submenu will appear on the right hand side. Here you can add all the available answers. Be sure to separate each answer with by hitting the return button:
